Acute Scrotum: Surgical Therapy

Caveat. Never accept a relevant loss of time. The guiding principle in the treatment of acute scrotum is: “Whenever doubt exists, it is safer to explore”, i.e., in case of doubt, surgical exposure of the testis.

The therapy of acute scrotum depends on the cause:

  • Testicular torsion – immediate surgical exposure of the testis with orchidopexy including the contralateral testis (“located on the opposite side or half of the body”) (one- or two-sided) or, if necessary, removal of the affected testis (see testicular torsion/surgical therapy for details).
  • Hydatid torsion – symptomatic therapy with bed rest, cooling, antiphlogistic measures; if necessary, also analgesics (painkillers); in individual cases, surgical therapy with ablation of the hydatids.
