- Do you suffer from symptoms such as fever, headache, irritability or photophobia?
- Do you suffer from nausea? Have you vomited?
- Have you had a seizure?*
- Have you noticed any signs of paralysis?*
- Do you have any dizzy spells?
- Have you noticed any skin changes?
- Have you noticed any changes in your behavior (irritability, confusion, lack of drive)?
- How long have these changes existed?
- Has the symptomatology worsened? In what period of time?
- Did you have contact with other ill persons?
- What was the last thing you ate?
Self history including medication history.
- Pre-existing conditions (neurological diseases, ENT diseases, infections (cold, earache); condition after head trauma / head injury?)
- Operations
- Vaccination status (Haemophilus influenzae)
- Allergies
Medication history
- Antibiotic use?
- Immunosuppression?
- Cytostatic drugs
