Blood in Ejaculate (Hemospermia): Test and Diagnosis

Laboratory parameters of the 1st order – obligatory laboratory tests.

  • Urine status (rapid test for: pH, leukocytes, nitrite, protein, glucose, blood), sediment, if necessary urine culture (pathogen detection and resistogram, i.e. testing of suitable antibiotics for sensitivity/resistance) and urine cytology; midstream urine.
  • If necessary, macroscopic and microscopic examination of the ejaculatory fluid – to distinguish true hemospermia from sperm discoloration due to another cause.

2nd order laboratory parameters – depending on the results of the medical history, physical examination, etc. – for differential diagnostic clarification (in recurrent hemospermia).

  • Small blood count
  • Differential blood count
  • Inflammatory parameter – CRP (C-reactive protein)
  • Coagulation parameters – PTT, Quick
  • PSA (prostate specific antigen) (in men > 40 years of age).
  • Uric acid
  • Urethral swab (urethral swab)
  • Ejaculate cultures – if urotuberculosis is suspected.
