Breast Cancer (Mammary Carcinoma): Therapy of Local Recurrence

Local recurrence means a recurrence of the tumor:

  • In the same mamma (breast) resp.
  • In the same-sided thoracic wall (chest wall) including the overlying skin, respectively.
  • In the regional lymph nodes of the axilla, the region around the clavicle or along the arteria mammaria interna vessels.

Note: In local recurrence, for example, in addition to in-sano resection, options for re-radiatio (radiotherapy) and re-induction of cystostatic drugs should be considered.

If breast-conserving therapy (BET)/surgery has already taken place, an ablatio mammae (mastectomy) must be performed in the event of a local recurrence (recurrence of a tumor at a previously treated site).

If the tumor recurs after ablation, it is removed over a large area with a tumor-free margin of about four centimeters. If necessary, a chest wall resection must be performed.

If radiotherapy has not been performed previously, it must now be performed as well:

  • Radiation of the thoracic wall and the draining lymphatic ducts.
  • Removal of individual metastases (daughter tumors) and post-irradiation of this region.
  • Systemic hormone therapy for extensive tumors that cannot be removed, lymphangiosis carcinomatosa or repeated recurrence.

If a radiatio (radiotherapy) has previously taken place, in individual cases, a new small-volume irradiation may be indicated or discussed in:

  • Unfavorable risk factors
  • Inoperability
