Carbetocin

Carbetocin is commercially available as a solution for injection (Pabal, generic). It has been approved in many countries since 2008.

Structure and properties

Carbetocin (C45H69N11O12S, Mr = 988.2 g/mol) is a synthetic octapeptide analog of oxytocin.

Effects

Carbetocin (ATC H01BB03) is a rapid and long-acting oxytocin agonist. It binds to oxytocin receptors in uterine smooth muscle, increasing muscle tone and stimulating contractions. It thus prevents uterine atony and excessive bleeding. Unlike oxytocin, it does not need to be infused for several hours because it has a longer duration of action.

Indications

To prevent uterine atony after cesarean delivery under anesthesia.

Dosage

According to the SmPC. The drug is injected intravenously once. Administration is slow and immediately after cesarean delivery preferably before removal of the placenta.

Adverse effects

The most common possible adverse effects include low blood pressure, facial flushing, a feeling of warmth, itching, headache, tremor, nausea, and abdominal pain.
