Carpal Tunnel Syndrome: Or something else? Differential Diagnosis

Cardiovascular System (I00-I99).

  • Raynaud’s syndrome (Raynaud’s disease) – vascular disease characterized by seizure-like paling of the hands or feet due to vasospasm.

Infectious and parasitic diseases (A00-B99).

Psyche – nervous system (F00-F99; G00-G99)

  • Polyneuropathy – pathological change of several nerves, leading mainly to paresthesias (insensations).
  • Polymyalgia rheumatica – disease belonging to the vasculitides (vascular inflammations) with acute pain of the shoulder and pelvic girdle muscles.
  • Pronator teres syndrome – syndrome affecting the pronator teres muscle; it involves part of the median nerve in the forearm.
  • Scalenus syndrome – compression syndrome affecting the neck; in this case, there is compression of the brachial plexus due to narrowing of the scalenus gap. The scalenus gap between Musculus scalenus anterior and medius can be severely narrowed by muscle hypertrophy , by a neck rib and by a steep or exostoses (= demarcated addition of compact bone substance (compacta) with outward growth) of the first rib.
  • Syringomyelia – neurological disease caused by the formation of cavities in the spinal cord.
  • Spinal muscular atrophy (SMA) – muscle atrophy resulting from a progressive decline of motor neurons (2nd motoneuron) in the spinal cord.
  • Thoracic-outlet syndrome (TOS; shoulder girdle compression syndrome) – temporary or permanent compression of a vascular nerve bundle consisting of brachial plexus, subclavian artery, and subclavian vein; considered one of the most controversial nerve compression syndromes
  • Forearm compartment syndrome – condition in which, with the skin and soft tissue sheath closed, increased tissue pressure leads to a decrease in tissue perfusion. This leads to neuromuscular disorders or tissue and organ damage. Most often, the compartment syndrome occurs on the forearm or lower leg.
  • Cervical myelopathy – damage to the spinal cord of the cervical spine, the cervical cord.
  • Cervical radiculopathy of the roots C6 and C7 – nerve root syndrome; irritation or damage to the nerve roots (radiculopathy) in the cervical spine (C-spine).
