Chronic Pain: Or something else? Differential Diagnosis

Diseases that can lead to chronic pain include:

Endocrine, nutritional, and metabolic diseases (E00-E90).

  • Fabry disease (synonyms: Fabry disease or Fabry-Anderson disease) – X-linked lysosomal storage disease due to a defect in the gene encoding the enzyme alpha-galactosidase A, resulting in progressive accumulation of the sphingolipid globotriaosylceramide in cells; mean age of manifestation: 3-10 years; early symptoms: Intermittent burning pain, decreased or absent sweat production, and gastrointestinal problems; if left untreated, progressive nephropathy (kidney disease) with proteinuria (increased excretion of protein in urine) and progressive renal failure (kidney weakness) and hypertrophic cardiomyopathy (HCM; disease of the heart muscle characterized by thickening of the heart muscle walls). Warning signs (red flags) that indicate Fabry disease:
    • Pain crises?
    • Pain in hands and feet of burning character?
    • Pain peripherally intensified (hands and feet) and of high intensity?
    • Pattern of spread of pain from distal to central?
    • Sweat secretion decreased and not adapted to physical exertion?
    • Trigger: heat or cold, exertion from exercise, stress or fever?
    • Family members with similar complaints?

Factors influencing health status and leading to health care utilization (Z00-Z99).

  • Chronic stress
  • Bullying

Musculoskeletal system and connective tissue (M00-M99)

  • Arthritis (inflammatory joint disease).
  • Osteoarthritis
  • Fibromyalgia (fibromyalgia syndrome) – syndrome that can lead to chronic pain (at least 3 months) in several regions of the body.
  • Rheumatological diseases

Neoplasms – tumor diseases (C00-D48)

  • Cancer, unspecified (breakthrough pain in tumor patients; BTCP, “breakthrough cancer pain”).

Psyche – nervous system (F00-F99; G00-G99).

  • Anxiety disorders
  • Depression
  • Diabetic neuropathy – chronic disorders of peripheral nerves or parts of nerves caused by diabetes mellitus (diabetes).
  • Mental disorders, without further specification
  • Somatic symptom disorder (SSD) associated with pain.

Symptoms and abnormal clinical and laboratory findings not elsewhere classified (R00-R99)

  • Pain, unspecified

Injuries, poisonings, and other consequences of external causes (S00-T98)

  • Fractures (bone fractures; hip and vertebral fractures) → Chronic pain in multiple body regions (“chronic widespread pain,” CWP): pain that persists for more than three months and affects the axial skeleton, the right and left sides of the body, and regions above and below the waist:
    • Vertebral fractures: 2.7-fold increase in CWP in men, 2.1-fold increase in women
    • Hip fractures in women: 2.2-fold CWP increase.
  • Trauma (injury), unspecified
