  • What symptoms have you noticed?
  • How long have these symptoms been present?
  • Do you have a fever? If so, how high is the fever? How long has it been present? Does the temperature fluctuate?
  • Have you noticed any change in the skin?
    • Skin rash? If so, where?
    • Bleeding into the skin* ?
  • Do you suffer from headaches, muscle and joint pain?
  • Have you been bitten by a mosquito recently?
