The diagnosis of a rash on the back includes an exact anamnesis of the patient, which mainly asks since when the rash has been present on the back, whether it is rather itchy or painful, whether similar complaints have been present before, whether there are accompanying symptoms such as fever or other signs of infection or whether shower gel, skin lotions or washing powder have been changed recently. Furthermore, a central role in diagnostics is played by the eye diagnosis.An experienced dermatologist can at least narrow down the causes of the complaints on the basis of the form, distribution and symptoms. If a fungal disease is suspected, the dermatologist will take a smear of the affected skin area and have it cultured in the laboratory. Here, skin fungi can be made visible and an appropriate antibiogram can be prepared. With the dermatoscope (a kind of magnifying glass with light) the dermatologist can get an enlarged overview of the affected skin area.

Therapy

The therapy depends on the presumed cause of the skin reaction. In case of uncertainty and thus also to be seen as a general measure, the omission of the potentially triggering substances should be considered. No matter whether a shower gel, a washing lotion or a washing powder has been changed or has been used for a long time – a change of the same should be made in any case to ensure that it is not an intolerance.

If a sun allergy is suspected, the sun should be avoided. Calcium or magnesium preparations can improve the symptoms or, in the best case, make them disappear completely. In case of allergic skin reactions, treatment with Fenistil® Gel can be attempted.

This is an anti-allergic substance that is supposed to symptomatically relieve the complaints in the area of the affected skin on the back. At first it does not matter what caused the allergy. The gel can be used especially if the skin area is limited, i.e. if only isolated reddish skin areas are visible.

As soon as larger areas are affected, a systemic anti-allergic treatment with a tablet should be considered. Antihistamines such as cetirizine or loratadine would be the treatment of choice. However, it should be noted that cetirizine can cause severe fatigue.

The symptoms should have improved after 7 days at the latest. Eczema of the back can also be treated with cortisone preparations. Here, attention should be paid to an initially low dosage, which could then also be given in high doses if necessary.

If a fungal disease of the skin is suspected, antimycotics in the form of creams or ointments would be used. If shingles is suspected, treatment with the viral agent acyclovir should be started as soon as possible. Many plant-based active ingredients are also well suited for the treatment of skin rashes, as they alleviate possible itching and have an anti-inflammatory effect.
