Dimercaptopropanesulfonic Acid (DMPS)

Dimercaptopropanesulfonic acid is commercially available in some countries as a solution for injection and in capsule form (Dimaval).

Structure and properties

Dimercaptopropanesulfonic acid or DMPS (C3H8O3S3, Mr = 188.3 g/mol) exists in the drug as a sodium salt and monohydrate. It is a dithiol and a sulfonic acid structurally related to dimercaprol.

Effects

DMPS (ATC V03AB43) binds to heavy metals via the sulfhydryl group (-SH) and forms stable complexes with them, which are excreted in the urine.

Indications

For the treatment of acute poisoning with mercury or lead. Other possible indications include arsenic, copper, antimony, chromium, and cobalt poisoning. The FOPH additionally mentions intoxication with the radionuclide polonium-210 as an indication in the antidote list.

Dosage

According to the SmPC. The solution for injection is administered intravenously or intramuscularly. The capsules are taken several times daily on an empty stomach.

Contraindications

DMPS is contraindicated in cases of hypersensitivity. Full precautions can be found in the drug label.

Interactions

Do not mix the solution for injection with other drugs because DMPS may be inactivated. DMPS inactivates essential trace elements, such as copper and zinc. Another interaction is possible with activated charcoal.

Adverse effects

The most common possible adverse effects include allergic skin reactions, chills, and fever. A drop in blood pressure may occur if the injection is given too quickly.
