Doripenem

Structure and properties

Doripenem (C15H24N4O6S2, Mr = 420.5 g/mol) exists as doripenem monohydrate, a white to slightly yellow crystalline powder. It bears a 1-β-methyl group that protects it from degradation by dehydropeptidase I.

Effects

Doripenem (ATC J01DH04) is bactericidal against numerous aerobic and anaerobic Gram-positive and Gram-negative bacteria. Its effects are based on inhibition of bacterial cell wall synthesis by binding to penicillin-binding proteins. Doripenem is stable to many beta-lactamases. It has a short half-life of about one hour.

Indications

For treatment of bacterial infectious diseases with susceptible pathogens.

Dosage

According to the SmPC. The drug is administered as an intravenous infusion.

Contraindications

Doripenem is contraindicated in case of hypersensitivity, including to other beta-lactam antibiotics. For complete precautions, see the drug label.

Interactions

Doripenem is actively secreted on the kidney, so interaction with probenecid is possible. Another drug interaction has been described with valproic acid.

Adverse effects

The most common possible adverse effects include headache, oral thrush, vaginal thrush, phlebitis, nausea, diarrhea, pruritus, and rash.
