In 99% of all cases, the liver is the primary target organ, where the six-hooked larva (oncosphere) undergoes metamorphosis to become a metacestode.

- Cholestasis (biliary stasis)
- Liver cirrhosis – connective tissue remodeling of the liver leading to functional impairment.
- Portal hypertension – increase in blood pressure in the portal vein.
- Pseudocysts of the liver – cyst-like structure in the liver, but which, unlike the cyst, has no epithelial lining

- Formation of secondary cysts in the pleura (lung pleura).
- Formation of secondary cysts in the peritoneum (peritoneum).

- Bacterial cholangitis (bile duct inflammation).
Injuries, poisonings, and other sequelae of external causes (S00-T98).
- Allergic reactions up to and including anaphylaxis (most severe form of allergic reaction; emergency)
