Exercises after a rotator cuff rupture – OP

A compromise must be found in the physiotherapeutic follow-up treatment: on the one hand, the suture must be sufficiently immobilized, on the other hand, no muscle atrophy (muscle atrophy) should occur or a stiff shoulder should develop. The most common strategy is to spare the arm for the first 10 days. In addition, the arm is immobilized on an abduction cushion at an angle of 45 degrees for the first 4-6 weeks.

Parallel to this, passive mobilization exercises are started by the physiotherapist. These improve the gliding ability of the tendon, increase the tensile strength and lead to better tissue nutrition. In the second step, active exercises begin, such as pendulum movements or gliding exercises on the table.

Then abduction (abduction), elevation (raising the arm) and rotation (turning) are practiced without resistance or strain. Exercises to build up strength, e.g. with light weights or the Thera band, only start from week 8-10. The aim is to stabilize the shoulder blade, center the head of humerus in the shoulder joint and rebuild strength and mobility of the entire shoulder musculature. Well suited are stabilizing exercises such as the quadruped stand or the wall support.If final everyday movements are possible, sport-specific exercises can be intensified from the 4th -9th month onwards. Wear processes, overstressing or accidents can result in a rotator cuff rupture. Those affected suffer from severe pain and restricted shoulder movement. Physiotherapeutic exercises may not heal the torn tendon, but they can help to reduce pain and improve mobility.

If the goal is not achieved, or if it is a fresh rupture caused by an accident, surgery is necessary. Various surgical techniques are used to reattach the torn tendon to the bone. Physiotherapeutic follow-up treatment is then necessary.
