Gum Replacement (Gum Epitheses)

A gum epithesis (synonym: gum replacement) is a gum mask made of soft-tissue silicone or rubber that replaces missing gums to improve aesthetics. The epithesis is removable and needs to be replaced after about a year. A common reason for gum loss is periodontitis. This disease of the periodontium causes the gums to recede. As a result, the necks of the teeth are exposed, pain occurs and the gaps between the teeth become unsightly larger. These gaps then lead to problems with pronunciation (S, Sh and Z sounds) and tongue dysfunction, such as sucking or pressing through the large interdental spaces. Moist pronunciation can also result.

After surgeries, such as the placement of implants, problems with gum regeneration may occur. Exposed metal crown and bridge margins negatively affect esthetics. The epithesis helps in this case as well.

Contraindications

The production

Epitheses are usually made for the visible area, that is, the anterior region. However, extension into the molar region (molars) is also possible.

Epitheses can be made only after periodontal treatment has been completed with stable results. The conservative treatment (filling therapy) in the affected area should also be completed.

The fabrication is completely individual. After the dentist has taken an impression – using silicone or polyether – of the gum area to be restored with the help of an individually made tray and has determined the color of the gum most accurately, the impression is sent to a laboratory. An epithesis is then made in the laboratory to protect the tooth necks and cover unsightly gaps. The epitheses are made in such a way that the transition to the natural gums is as invisible as possible. The epithesis can be removed daily for cleaning. It should be cleaned under running water after eating. Due to discoloration caused by food and drinks, which also occurs on natural teeth, an epithesis usually needs to be replaced after one year. After brushing teeth with fluoridated toothpaste, the epithesis should not be reinserted directly, as this will cause yellowish discoloration. It is also advisable to wait a little before inserting the epithesis after eating carrots in order to avoid rapid yellow discoloration.

The epithesis should not be worn at night.
