Severe or refractory cases may benefit from a combination of HBOT with glucocorticoid therapy; authors recommended for this purpose: 100% oxygen at a pressure of 2.0 to 2.5 bar, either for two weeks at 90 minutes daily or alternatively 20 days at 60 minutes each; combination therapy resulted in patients in this group hearing significantly better (OR 1.43), as well as an increased chance of fully recovering original hearing (OR 1.61).
The timing of the start of hyperbaric oxygen therapy seems to be decisive for the success of therapy (absolute and relative hearing gain): so according to the study results, the start of therapy should be within 24 to 48 hours after hearing loss.
