General measures
- Individuals with hemophilia should obtain an identification card and carry it with them at all times
- Injections should be administered intravenously and/or subcutaneously
- Very careful hemostasis should always be performed after injuries/surgeries
The following agents should be avoided:
- Antiplatelet agents (TAH) [There is no claim to completeness!]
- Abciximab
- Acetylsalicylic acid (ASA)
- Combination of acetylsalicylic acid and dipyridamole.
- Clopidogrel
- Eptifibatide
- Ilomedin (prostacyclin analogue): prostacyclin is the most potent endogenous platelet aggregation inhibitor (drugs that inhibit platelet clumping (platelet aggregation)).
- Prasugrel
- Ticagrelor
- Ticlopidine
- Tirofiban
- Non-steroidal anti-inflammatory drugs (NSAIDs; anti-inflammatory drugs not derived from sterols) such as ibuprofen.
- Oral anticoagulants (anticoagulants) such as phenprocoumon (coumarin derivative).
- Hirudine
- Heparins, heparinoids
The following analgesics (pain relievers) may be used:
- Tilidine/naloxone
- Buprenorphine
- Paracetamol
- Diclofenac
Conventional non-surgical therapy methods
- Gene therapy for hemophilia B (first positive therapy trials).
Vaccinations
The vaccinations recommended by the STIKO should be given. However, these should not be administered intramuscularly without factor substitution.
Sports Medicine
- Endurance training (cardio training) and strength training (muscle training).
- The trained muscles protect the joints.
- High risk sports (ie not suitable for hemophilia):
- Team sports with a lot of physical contact, regular impacts on the legs and high risk of falls/injuries, such as basketball, (ice) field hockey, soccer, soccer, handball,
- Medium risk sports (suitable with precautions):
- Individual sports (such as inline skating, cycling, golf, dancing, tennis sailing) where the risk of injury can be minimized by precautions, such as a helmet.
- Recommended sports:
- Preparation of a fitness or training plan with suitable sports disciplines based on a medical check (health check or athlete check).
- Detailed information on sports medicine you will receive from us.
Physical therapy (including physiotherapy)
- Adjunctive physiotherapy is required for joint hemorrhage.
