Hepatic Encephalopathy: Test and Diagnosis

1st order laboratory parameters – obligatory laboratory tests.

  • Ammonia – liver detoxification performance parameter [ammonia ↑]
  • Small blood count
  • Inflammatory parameter – CRP (C-reactive protein)
  • Electrolytescalcium, chloride, potassium, magnesium, sodium, phosphate.
  • Blood gas analysis (BGA) with lactate determination.
  • Serum globulins
  • Serum albumin – important protein (protein) [albumin ↓, as a sign of liver synthesis disorder].
  • Liver parameters – alanine aminotransferase (ALT, GPT), aspartate aminotransferase (AST, GOT), glutamate dehydrogenase (GLDH) and gamma-glutamyl transferase (gamma-GT, GGT), alkaline phosphatase, bilirubin.
  • CHE (cholinesterase) [CHE ↓, as a sign of liver synthesis disorder]
  • Renal parameters – urea, creatinine, cystatin C or creatinine clearance, as appropriate.
  • Fasting glucose (fasting blood glucose), if necessary oral glucose tolerance test (oGTT) – to exclude hypoglycemia (hypoglycemia) or diabetes mellitus disease.
  • Coagulation parameters – PTT, Quick
  • Vitamin B1 – due topossible Wernicke’s encephalopathy? (degenerative encephaloneuropathic disease of the brain in adulthood; clinical picture: brain-organic psychosyndrome (HOPS) with memory loss, psychosis, confusion, apathy, as well as gait and stance unsteadiness (cerebellar ataxia) and eye movement disorders / eye muscle paralysis (horizontal nystagmus, anisocoria, diplopia)); vitamin B1 deficiency (thiamine deficiency)).
