Hydrotalcite

Hydrotalcite was approved in many countries since 1992 and was commercially available as a suspension (Rennie Gel Hydrotalcite, off label). In Germany, it is also available as chewable tablets (Talcid, generic).

Structure and properties

Hydrotalcite (Al2Mg6(OH)16CO3 – 4H2O, Mr = 531.9 g/mol) is a magnesium aluminum hydroxide carbonate hydrate with a layered lattice structure. The natural substance exists as a white powder and is practically insoluble in water.

Effects

Hydrotalcite (ATC A02AD04) neutralizes stomach acid, binds pepsin and bile acids, and thus relieves acid-related disorders. The effect is rapid and lasts up to 1.5 hours. Hydrotalcite is mainly effective in the digestive tract. However, small amounts of the magnesium and aluminum may be absorbed into the body.

Indications

For the treatment of gastric complaints of hyperacidity, burning of the stomach, and acid regurgitation.

Dosage

According to the package leaflet. The medicine is taken between meals, usually one to two hours after eating or as needed.

Contraindications

  • Hypersensitivity
  • Impaired kidney function
  • Muscle weakness
  • Hypophosphatemia

Interactions

Hydrotalcite may reduce the absorption of other drugs (e.g., antibiotics) and should therefore be given at a time interval from them. Alkalinization of the urine may affect the excretion of other drugs. Acidic beverages should not be consumed at the same time because this may increase aluminum absorption.

Adverse effects

Possible adverse effects include soft stools, diarrhea, and vomiting. With prolonged use, disturbances in phosphate and calcium balance cannot be ruled out.
