Isradipine

Products

Isradipine is commercially available in capsule form (Lomir SRO). It has been approved in many countries since 1991.

Structure and properties

Isradipine (C19H21N3O5, Mr = 371.4 g/mol) is a racemate. It exists as a yellow crystalline powder that is practically insoluble in water.

Effects

Isradipine (ATC C08CA03) has antihypertensive properties. Its effects are due to blockade of L-type voltage-gated calcium channels in vascular smooth muscle. As a result, it triggers peripheral vasodilatation.

Indications

For the treatment of high blood pressure (essential hypertension).

Dosage

According to the SmPC. The capsules are usually taken once daily, regardless of meals.

Contraindications

  • Hypersensitivity, including to other dihydropyridines.
  • Cardiogenic shock
  • Unstable angina
  • During a myocardial infarction and during one month afterwards.

Interactions

Isradipine is metabolized by CYP3A4. Corresponding drug interactions with CYP inhibitors and inducers are possible. Other antihypertensive agents may potentiate the effects of isradipine.

Adverse Effects

The most common potential adverse effects include headache, peripheral edema, warm sensation, and flushing. Other common side effects include rash, breathing difficulties, fatigue, malaise, increased urine output, dizziness, indigestion, rapid pulse, and palpable heartbeats.
