Lymphogranuloma Venereum: Test and Diagnosis

1st order laboratory parameters – obligatory laboratory tests.

  • Microscopic of the bacterial species Chlamydia trachomatis by immunofluorescence.
  • Detection of serotypes L1-L3 of the bacterial species Chlamydia trachomatis [acute LGV infection fourfold increase in antibody titers; CFT > 1:64, MIFT > 1:128].
  • DNA detection of Chlamydia trachomatis by “nucleic acid amplification tests” (NAAT) (gold standard; sensitivity of circa 90% and a specificity of 100%.
  • HIV test (in the case of unknown HIV status).

Examination material: vesicle contents of the primary lesion; ulcer material of anogenital lesions, smears (genital or rectal), rectal biopsies in case of anorectal involvement, lymph node aspirate.

2nd order laboratory parameters – depending on the results of the history, physical examination, etc. – for differential diagnostic clarification

  • Bacteria
    • Neisseria gonorrhoeae (gonorrhea, gonorrhea) – genital swab for pathogen and resistance, specifically for Neisseria gonorrhoeae.
    • Treponema pallidum (syphilis, lues) – antibodies against Treponema pallidum (TPHA, VDRL, etc.).
    • Ureaplasma urealyticum
  • Viruses
    • HIV (AIDS)
    • Herpes simplex virus type 1/2 (HSV type 1 u. 2)
  • Mycoses / parasites
    • Candida albicans a.o. Candida species genital smear – pathogen and resistance).
    • Trichomonas vaginalis (trichomoniasis, colpitis) – antigen detection.
