Nedocromil

Products

Medicines containing nedocromil are no longer available in many countries. The eye drops (Tilavist), nasal spray (Tilarin), and preparation for inhalation (Tilade) are out of commerce, presumably for commercial reasons. This article refers to ocular use.

Structure and properties

Nedocromil (C19H17NO7, Mr = 371.34 g/mol) belongs to the class of pyranoquinolines and is present in drugs as a yellowish disodium salt. Nedocromil was originally developed for the therapy of bronchial asthma.

Effects

Nedocromil (ATC S01GX04) has anti-inflammatory and antiallergic effects. It is a mast cell stabilizer that inhibits the release of inflammatory mediators from eosinophil and neutrophil granulocytes and mast cells. The effects take a few days to appear. The effects are due to the inhibition of chloride ion channels in the conjunctiva that are involved in mediator release.

Indications

For the treatment of seasonal and nonseasonal allergic conjunctivitis.

Dosage

According to the professional information. 1 drop is placed in both eyes 2-4 times daily.

Contraindications

Nedocromil is contraindicated in hypersensitivity and in children under 6 years of age. Soft contract lenses should not be worn during treatment with Nedocromil drops. The preservative benzalkonium chloride lodges in soft lenses and may cause corneal damage due to prolonged release.

Interactions

Silver-containing eye drops may form insoluble salts with nedocromil and result in attenuation of effect.

Adverse effects

Local irritation may occur that resolves quickly. Some patents find a bitter taste unpleasant.
