Nose Correction (Rhinoplasty)

The nose is one of the parts of the body that many people are unhappy with. It is too big, too long, crooked or carries an unsightly hump. Unfortunately, you can not hide or conceal the nose.Often suffer from their nose and are under great psychological pressure.A nose job is the most sensible solution. The shape of the nose is harmoniously adapted to the face and you get a whole new attitude to life. The nose can be surgically changed in size, shape, length and width.About the history of nose correctionFirst nose operations were performed centuries ago. People who had committed a crime had their nose cut off at that time so that it was obvious to everyone if someone had been a criminal. The first nose replacement surgeries were performed in India at that time.

Indications (areas of application)

  • Congenital shape changes such as a hump nose.
  • Injury consequences such as crooked nose and saddle nose.
  • Desire for nose tip or nose wing changes in the context of cosmetic surgery

Notice:

  • The operation should be avoided as much as possible in the growth phase – for example, in congenital or acquired crooked nose.
  • In the case of a body schema disorder, ie a normal nose is perceived as too large, conspicuous and problematic, a specialist should clarify the psychosomatic background.

Before surgery

Before surgery, an appropriate endoscopic examination of the internal nose should be performed. The patient should not take acetylsalicylic acid (ASA), sleeping pills or alcohol for a period of seven to ten days before surgery. Both acetylsalicylic acid and other painkillers delay blood clotting and can cause unwanted bleeding. Smokers should severely limit their nicotine consumption as early as four weeks before the procedure to avoid jeopardizing wound healing. No respiratory illnesses should be present at the time of surgery.

The surgical procedure

Depending on the extent of the corrections, the procedure is either performed on an outpatient basis, so that you can go home afterwards, or you will have to stay in the clinic for a few days. Before the start of the operation, the desired goal is discussed with the help of photos or by means of a computer program. The incisions in a nose correction are usually located inside the nose. Sometimes small external incisions are required, depending on the individual situation, but these also leave barely visible scars. Then the mucosa must be separated from the nasal cartilage and bone. Then the real art begins – the nose correction. The surgeon removes cartilage and bone and thus shapes a new nose.A nose that is too small can be enlarged by implanting cartilage or bone tissue or even an artificial implant. After the shaping is complete, the mucous membranes that supply the nose are reattached and sutured with tiny sutures. The nose is then splinted using plaster, metal or plastic to stabilize the new shape. Tamponades (soft tubes) are inserted inside the nose, which also serve to protect and maintain the shape of the new nose. The procedure is performed under local anesthesia (local anesthesia) or general anesthesia (anesthesia).

After the surgery

As after most cosmetic surgery procedures, there will be swelling and bruising that will subside within about two weeks. It will take about a month for the nose to become stable in its new shape. Be extra vigilant during this time. A final result is achieved after about six months.

Possible complications

  • Swelling and hematoma (bruising) usually occur
  • Wound healing disorders as well as infections in the area of the incision, especially in the case of incisions on the columella
  • External incisions can form scars, which, however, usually fade and become tender; here, if necessary, in wound healing disorders or in case of predisposition also keloids (bulging scars) and / or skin discoloration can occur (rare)
  • Post-bleeding from the nose (rare).
  • Infections of the soft tissues, cartilage and bone (rare).
  • Injuries are extremely rare!
    • Of the olfactory nerves (→ dysosmia / olfactory disorders).
    • Of the sensory nerve (→ feeling of fuzziness in the cheeks).
    • of the tear duct (→ eye tears)
    • Of the skull base with leakage of cerebrospinal fluid (brain water).
  • Implanted cartilage can bend or be broken down by the body
  • Correction of the nasal septum (nasal septal wall) may later result in a renewed nasal septum deviation (nasal septal wall curvature); occasionally, a septal perforation (hole) is also possible, resulting in a connection of the right and left nasal cavities, with the consequence of a whistling sound when breathing
  • When correcting the bone, tooth damage is possible (rare).
  • As after any surgical procedure, thrombosis (blood clot in a vessel) may occur, with the possible consequence of embolism (occlusion of a blood vessel) and thus pulmonary embolism (danger to life).
  • The use of electrical devices (e.g. electrocoagulation) can cause leakage currents, which can lead to skin and tissue damage.
  • Positioning on the operating table may cause positional damage (e.g., pressure damage to soft tissues or even nerves, resulting in sensory disturbances; in rare cases, this may also lead to paresis of the affected limb).
  • In case of hypersensitivity or allergies (e.g. anesthetics/anesthetics, medications, etc.), the following symptoms may temporarily occur: Swelling, rash, itching, sneezing, watery eyes, dizziness or vomiting.
  • When using local anesthetics (local anesthesia) in the facial area, in extremely rare cases, occlusion of central optic nerve vessels and consequent visual deterioration to loss of vision is possible.
  • Infections, after which severe life-threatening complications concerning heart, circulation, respiration, etc. occur, are very rare. Similarly, permanent damage (eg, paralysis) and life-threatening complications (eg, sepsis / blood poisoning) after infections are very rare.
