General measures

  • Aim for normal weight! Diet and exercise should be at the forefront of therapeutic measures! Often, weight reduction alone already leads to a normalization of the cycle and follicle maturation (egg maturation); significant improvements are seen in follicle-stimulating hormone (FSH), sex hormone-binding globulin (SHBG), total testosterone, androstenedione, free androgen index and FG score (Ferriman-Gallwey score for quantifying hirsutism/increased androgen-dependent hairiness). Determination of BMI (body mass index) or body composition using electrical impedance analysis.
    • BMI ≥ 25 → participation in a medically supervised weight loss program.
  • Review of permanent medication due topossible impact on the existing disease.

Nutritional medicine

  • Nutritional counseling based on nutritional analysis
  • Women with polycystic ovary syndrome often have metabolic syndrome, meaning that three of the following five criteria are present together: Obesity, hypertension (high blood pressure), elevated triglycerides (> 150 mg/dl), lowered HDL cholesterol (< 50 mg/dl) in the blood, diabetes mellitus type 2. The following recommendations should be prioritized for women with metabolic syndrome:
    • Energy-reduced mixed diet
    • Distribute food over 3 meals a day, no snacks between meals
    • Low-carbohydrate, high-protein and fat-modified diet – selection of foods that lead to low blood glucose serum level (blood sugar level) and low insulin secretion.
    • High-fiber diet (whole grain products).
    • Daily total of 5 servings of fresh vegetables and fruits (≥ 400 g; 3 servings of vegetables and 2 servings of fruit).
    • Slow and deliberate chewing, so that a feeling of satiety can arise
  • Selection of appropriate food based on the nutritional analysis

Sports Medicine

  • Endurance training (cardio training) and strength training (muscle training) → sports activity is an important measure for weight reduction and leads to maintaining the weight permanently afterwards
