Viagra is a prescription sexual enhancer. It is used to treat erectile dysfunction and in some cases in idiopathic pulmonary arterial hypertension. The active substance is sildenafil.

Sildenafil, like tadalafil (Cialis), is a phosphodiesterase-5 inhibitor. Accordingly, it also acts like Cialis, in which the inhibition of PDE-5, with appropriate sexual stimulation, leads to a faster and longer erection. Sildenafil was the first in a class of PDE-5 inhibitors and has gained wide recognition since its introduction as Viagra.

Originally, it was intended to be used for the treatment of high blood pressure. Its effect in erectile dysfunction was discovered accidentally. Like Cialis, it is contraindicated in severe cardiovascular diseases, such as coronary heart disease or heart failure.

The exact dosage and the duration of the therapy should be determined by a doctor. Yohimbine is a drug that is used to treat erectile dysfunction. It is extracted from the bark of the yohimbe tree, which is found mainly in West Africa.

It is taken orally in the form of tablets. It acts as an antagonist on the alpha2-adrenoreceptors. These receptors are located in blood vessels, among other things, and normally lead to vasoconstriction through the action of e.g. adrenaline.

However, if yohimbine now acts at these receptors, vasodilation occurs. This allows more blood to flow through the blood vessels, including the erectile tissue of the penis, and makes it easier to achieve an erection. There is also an increase in libido.

However, yohimbine is also able to cross the blood-brain barrier and thus act in the central nervous system. This leads to effects such as an increase in heart rate and blood pressure and increased motor excitability, up to and including tremors, which are rather undesirable. Other possible side effects are among others: headache, diarrhea, sleep disturbances, anxiety, restlessness, panic attacks, nausea, vomiting, nervousness, stomach pain, loss of appetite, increased sweating, bronchospasm and flashbacks. Although yohimbine may help some patients, its unreliable action and the availability of more potent PDE-5 inhibitors such as Viagra and Cialis have relegated it to the background in the treatment of erectile dysfunction.
