  • Do you work/live in a community facility?

Current medical history/systemic history (somatic and psychological complaints).

  • Do you suffer from severe vomiting and/or diarrhea?
  • What does the diarrhea look like? Watery, mucousy, bloody?
  • How long have these symptoms been present?
  • Have you noticed any other symptoms? Fatigue, muscle pain, headache?
