Scaphoid Fracture: Classification

Classification of scaphoid fracture according to Herbert and Fischer.

Type Type description
Type A Avulsion fracture or only cortical bone affected
Type B Longitudinal/transverse fracture:

  • B1 – oblique/center
  • B2 – transverse / center
  • B3 – proximal
  • B4 – luxation fracture
  • B5 – several larger fragments
Type C Multi-fragment fracture
Type D Pseudarthrosis (impaired fracture healing with the development of a false joint):

  • D1 – tight
  • D2 – mobile

Classification of scaphoid fracture according to Krimmer following Herbert (see above), taking into account the computed tomography findings.

Type Type description
Type A Stable fractures
A1 Tubercle fractures
A2 Undislocated fractures with transverse process in the middle or distal third
Type B Unstable fractures
B1 Long oblique fractures
B2 Dislocated or gaping fractures
B3 Fractures of the proximal third
B4 Transscaphoid perilunate dislocation fracture
