Secnidazole was approved in the United States in 2017 in granular form (Solosec). The substance is not new; it was developed in the 20th century.

Structure and properties

Secnidazole (C7H11N3O3, Mr = 185.2 g/mol) is an imidazole derivative (nitroimidazole) nitrated at position 5. It is a prodrug that is activated in bacteria.

Effects

Secnidazole has antibacterial and antiparasitic properties against most pathogens that cause bacterial vaginosis. These include and . In bacteria, radical anions are formed by enzymes from secnidazole, which interfere with DNA synthesis. The active ingredient has a long half-life of about 17 hours.

Indications

For the treatment of bacterial vaginosis in adult women.

Dosage

According to the professional information. The granules are sprinkled on applesauce, yogurt, or pudding, for example, and taken as a single dose. It does not dissolve and is not intended to be taken with water. Administration is independent of meals. The granules should not be chewed.

Secnidazole is a substrate of CYP450 isozymes.

Adverse effects

Possible adverse effects include vaginal thrush, headache, and gastrointestinal disturbances such as nausea, taste disturbances, vomiting, diarrhea, and abdominal pain.
