Side effects

Glitazones may cause heart failure after prolonged use in patients at risk or worsen in patients with pre-existing heart failure. Unfortunately, heart failure is particularly common in patients with type 2 diabetes. Doctors are therefore cautious and will not prescribe glitazones in patients with existing heart failure.

Heart weakness becomes noticeable through water retention, e.g. in the legs or through shortness of breath. Your doctor will regularly ask you about this during check-ups. An increase in weight as well as an increase in cholesterol has been observed in studies with many patients.

Glitazones can worsen liver values and cause discomfort in arms and legs. As with all other oral antidiabetics, Glitazones can cause gastrointestinal problems. In addition, signs of anemia have been observed with pioglitazone (Actos®) and rosiglitazone (Avandia®) therapy.

Contraindications

In cases of extreme overweight, glitazones increase weight gain and should therefore be replaced by other drugs. If you suffer from heart failure (cardiac insufficiency) or liver disease, you should not take Glitazone. If you are already injecting insulin, none of the Glitazone products are suitable for you. In the case of severe kidney disease or if regular dialysis treatment is necessary, a risk-benefit ratio under therapy with pioglitazone (Actos®) or rosiglitazone (Avandia®) should be carefully considered.

Interactions

If you are taking anti-inflammatory or pain medication in the context of a rheumatic disease, joint inflammation or arthrosis (NSAIDs, diclofenac, indometacin, piroxicam, ibuprofen), water retention in the body tissue and heart failure are promoted. Some chemotherapeutics inhibit the breakdown of rosiglitaton and increase its effect in the sense of hypoglycaemia. If you are taking medication against elevated blood lipid values, the effect of rosiglitazone can be enhanced.
