Tadalafil therapy (phosphodiesterase-5 inhibitor) is used to treat erectile dysfunction (ED) with medication.
Indications (areas of application)
- Erectile dysfunction
Contraindications
- Hypersensitivity to the active substance tadalafil.
- Taking nitrates or other nitric oxide donors.
- Uncontrolled arrhythmias (cardiac arrhythmias).
- Non-arteritic anterior ischemic optic neuropathy.
- Severe cardiac disease (for example, unstable angina, heart failure (> Grade I), coronary artery disease, cardiac arrhythmias, or stenosis around the heart valves)
- Severe hepatic insufficiency
- Uncontrolled arterial hypertension (high blood pressure).
- Hypotension (low blood pressure < 90/50 mmHg).
- Recent myocardial infarction (heart attack) – in the previous 90 days – or apoplexy (stroke) – in the previous 6 months).
- Diseases of the retina (retina)
In the following diseases, tadalafil should be used only after weighing the benefits against the risks:
- Severe hepatic insufficiency
- Anatomical deformities of the penis
- Glucose-galactose malabsorption
- Hereditary galactose intolerance
- Lactase deficiency
- Sickle cell anemia (med: drepanocytosis; also sickle cell anemia, sickle cell anemia) – genetic disease of erythrocytes (red blood cells); it belongs to the group of hemoglobinopathies (disorders of hemoglobin; formation of an irregular hemoglobin called sickle cell hemoglobin, HbS).
- Multiple myeloma (plasmocytoma).
- Leukemia (blood cancer)
- Taking alpha blockers such as doxazosin (cannot be recommended).
- Other treatments for erectile dysfunction (no studies in combination).

Possible side effects
- Common: headache (7%), facial flushing (4.6%), nasal congestion (5%).
- Occasional: dizziness, somnolence (lightheadedness), drowsiness), sinus pain, blurred vision, palpitations (heart palpitations), flushing, exertional dyspnea (shortness of breath on exertion), dyspepsia (irritable stomach), nausea, vomiting
