Topotecan is commercially available in capsule form and as a lyophilizate (Hycamtin, generic). It has been approved in many countries since 1996.

Structure and properties

Topotecan (C23H23N3O5, Mr = 421.4 g/mol) is present in the drug as topotecan hydrochloride. It is a semisynthetic derivative of camptothecin, a plant alkaloid derived from the tree.

Effects

Topotecan (ATC L01XX17) has cytotoxic and antitumor properties. The effects are due to selective inhibition of topoisomerase I. This causes single-strand breaks in DNA, eventually leading to cancer cell death.

Interactions

Topotecan is a substrate of BCRP and P-glycoprotein. Corresponding interactions with inducers and inhibitors are possible.

Adverse effects

The most common possible adverse effects include neutropenia, anemia, thrombocytopenia, nausea, diarrhea, vomiting, hair loss, and fatigue.
