The following therapeutic measures can be carried out for “cervical spine trauma grade 1 to 2”. Please note that there is a very slow but steady improvement in this clinical picture.
General measures
- In the early stages, if necessary” cold compress”; later, local heat or massage
- Early activating conservative treatment; prolonged immobilization is prognostically unfavorable.

Physical therapy (including physiotherapy)
- Physical therapy:
- Physiotherapy probably provides greater benefit than immobilization by means of a cervical collar (if immobilization, then a maximum of 5-10 days!)S1 Guideline: application of a Schanz collar or other mechanical immobilization (usually superfluous, exception: instability, massive pain on movement)
- Gentle active movement and loosening exercises.
- Intensive 12-week physical therapy combined with cognitive behavioral therapy (CBT) did not relieve symptoms of patients with chronic whiplash better than one-hour counseling in a comparative study
