Foreskin Hypertrophy, Phimosis and Paraphimosis: Therapy

General Measures

Conventional non-surgical therapy methods

  • Stretching treatment for phimosis/foreskin stenosis (first-line therapy) Procedure as follows: Retraction of the prepuce (foreskin) to release adhesions (adhesions):
    • Foreskin – once or twice a day for about 5-10 minutes – carefully pull back with two fingers as far as this is possible without pain and without the use of force (ie, gently).
    • Foreskin left in this position for a few seconds; then let the foreskin slide forward again or carefully push forward.

    Period: 4 weeks; stretching treatment in combination with topical (local; external) application of a corticosteroid cream (cortisone-containing cream applied about 4-5 minutes before stretching; e.g., betamethasone 0.1%; mometasone furoate 0.1%) during the first two weeks. Approximately 30% of boys can be permanently cured in this way.

In the case of phimosis (narrowing of the foreskin) that does not cause symptoms, definitive therapy can be waited until the end of puberty.
