Discharge from the hospital

Since October 2017, the hospital has been obliged under the so-called “discharge management” (which is also called “care or transition management”) to initiate any necessary follow-up care after hospitalization.

In this context, the hospital organizes, for example, outpatient rehabilitation measures after the hospital stay or also inpatient follow-up treatment (inpatient rehab).

At the time of discharge, home help can also be organized and – if necessary – a transition to a nursing home.

However, all these measures are not obligatory for you as a patient: you can also refuse these assistance measures – which you must confirm with a signature.
