Early Detection Examinations (U3–U11)

Early detection examinations (U3–U11) are a procedure for diagnostic testing of both the child’s physical and mental condition. The purpose of this examination is to determine if the developmental status is age appropriate and can be judged as adequate. The early detection program U3 to U9 comprises seven examination appointments up to the age of 6. Thus, the child is regularly examined and its development is monitored until it starts school. The early detection examinations U3 to U9 serve on the one hand to provide parents with information, to take a medical history, to make an orienting assessment of development, to conduct a clinical examination and to assess parent-child interaction, and on the other hand to provide parent counseling. The child screening examinations are performed by pediatricians and adolescent physicians, so that a high level of professional competence can be guaranteed in the examination of children and adolescents. If a physical or mental disorder of the child is diagnosed during the screening, there is a possibility of quick therapeutic intervention on the part of the doctor. This provides the opportunity to take advantage of early therapy, so that the risk of possible consequential damage can be reduced. In addition to checking the mental and physical condition, an assessment of any existing neglect or maltreatment of the child can also be made during a child check-up. Furthermore, it is part of the attending physician’s responsibilities to document attendance at a child screening examination in the child examination booklet.

Examination Time Services
U3 4-5 weeks of life
  • Determination of body weight, height and head circumference (U1-U9).
  • In-depth physical examination, inspection (viewing), etc. (see below U2)
  • Eye examination (see below U2).
  • Orientation assessment of development: can the baby already hold its head in the prone position, open its hands spontaneously or look attentively at nearby faces? Turn head toward sources of sound? Pronounced sucking and grasping reflex?
  • Sonography (ultrasound) of the infant hip (hip sonography) – screening procedure for early detection of hip maturation disorders and congenital deformities of the infant hip.
  • Stool color chart (U2-U4) – see below U2.
  • Vaccination advice: (from U3): vaccinations with suggestion for vaccination dates.
  • Anticipatory advice: dealing with writing baby, sudden infant death syndrome, accident prevention; rickets and caries prophylaxis; breastfeeding / nutrition / oral hygiene, regional support services (eg, parent-child assistance, early help). Vitamin K prophylaxis is checked and carried out if necessary.
  • Note: Tests for inborn errors of metabolism and / or cystic fibrosis and the newborn hearing test should be done now, if not already done.
U4 3rd-4th month of life
  • Determination of body weight, height and head circumference (U1-U9).
  • In-depth physical examination, inspection, etc. (see below U2)
  • Orienting assessment of development: examination in relation to physical and mental development of the child (see child examination booklet for U4).
  • Stool color chart (U2-U4) – see U2 below.
  • Repeat vaccinations; at the latest now the first vaccinations take place.

U5 6-7 months of life
  • Determination of body weight, body length and head circumference (U1-U9).

U7 21-24 months of life
  • Determination of body weight, body length and head circumference (U1-U9).
  • In-depth physical examination; inspection of the whole body in the supine and prone positions in the sitting position, from behind and from the sides; passive mobility of the major joints.
  • Orientation assessment of development:
    • The toddler should be able to speak at least ten words, understand 250 words and combine two words correctly.
    • Building a tower with four to eight blocks – left and right handed.
    • Child should be able to kick a ball left and right without holding on
  • Vaccination advice: have the basic vaccinations been completed? At the latest at U7, the second vaccination against measles, mumps, rubella and chickenpox should be carried out.

U7a 34th-36th month of life

  • Eye examination:
    • Binocular and stereo test (spatial vision test): Titmus, TNO test.
    • Vision test: Sheridan-Gardiner
    • Vision test with bilateral and right/left vision with occlusion of one eye.

U8 46-48 months of age
  • Determination of body weight, body length and head circumference (U1-U9).
  • In-depth physical examination: gait pattern, Romberg 10 seconds, one-leg stand 3 seconds.
  • Orienting assessment of development: can the child speak in the first person? Pronunciation disorders (eg stammering)?, behavioral abnormalities?, Motor clumsiness.
  • Hearing screening: only for children who develop a hearing disorder in the first years of life, hearing screening with the screening audiometer with five frequencies is mandatory at U8.
  • Immunization counseling: vaccination status is checked, and the doctor discusses with the mother / father further dates for booster vaccinations.
  • Anticipatory counseling: nutrition and exercise of the child, measures for accident prevention, the promotion of language development and the responsible use of media (eg TV, Internet, game consoles and the like) in the everyday life of the child.
U9 60th-64th month of life
  • Last screening examination before starting school
