Magnesium orotate is commercially available as a monopreparation in tablet form (e.g., Burgerstein Magnesium Orotate).
Structure and properties
Magnesium orotate (C10H6MgN4O8, Mr = 334.5 g/mol) is the magnesium salt of orotic acid. Orotic acid is a pyrimidine derivative. Magnesium orotate is usually present in drugs as magnesium orotate dihydrate. 400 mg magnesium orotate dihydrate is equivalent to 25 mg elemental magnesium.
Effects
Magnesium orotate (ATC A12CC09) is thought to have muscle-relaxing, relaxing, and cardioprotective properties.
Indications for use
- Calf cramps, muscle spasms.
- Magnesium deficiency
- As a general sedative
- As an adjuvant in heart failure (see, for example, Stepura & Martynow, 2009) – no official medical indication.

According to the instructions for use.
Contraindications
Contraindications include:
- Hypersensitivity
- Severe renal dysfunction
- Dehydration
- Mysthenia gravis
- AV block
- Predisposition to urinary tract infections with formation of magnesium ammonium phosphate stones.

Interactions
Magnesium orotate may interfere with the absorption of other drugs, such as tetracyclines, iron, and sodium fluoride. Therefore, an interval of 2 to 3 hours should be allowed between doses. Simultaneous use of cholecalciferol (vitamin D3) may increase calcium levels.
Adverse effects
Possible adverse effects include soft stools and diarrhea.
