What to organize:
- Intake plan for medication
- Have home, plants, animals taken care of – make notes for helpers and put them down where they can be easily seen
- Leave important phone numbers (eg veterinarian).
- Have mail, newspaper and other deliveries stored, cancel or reroute.
- Take care of payments due
- Cancel or postpone appointments
- Mark personal things
- Organize round trip
This goes in the clinic bag:
- Referral slip from the family doctor and insurance card.
- Documents of the general practitioner (e.g. X-rays, laboratory findings, etc.)
- Permanent medication with intake schedule
- Pacemaker card, allergy passport, Marcumar passport….
- Towels, washcloths, soap / shower gel, skin cream.
- Toothbrush stuff
- Brush, comb, nail scissors and file, razor.
- Linen and pajamas / nightgowns to change.
- Bathrobe, house suit
- Solid slippers
- Stationery, addresses, reading material
- Hearing aid, contact lenses, reading glasses, walking stick
- Smaller amount of money for the statutory co-payment and for prepaid cards (telephone, TV).
This stays at home:
- Larger amounts of money, valuables, jewelry.
- Cell phone (ask if it is allowed in the clinic).
