Alprostadil for Erectile Dysfunction

Alprostadil therapy is a drug treatment for erectile dysfunction (ED).Alprostadil is available for local (“topical”) and intraurethral (“into the urethra“) therapy.

Indications (areas of application)

Only after careful risk-benefit assessment by the treating physician should the agent be used for:

Contraindications

  • Hypersensitivity to the active substance
  • Severe heart disease (e.g., unstable angina, heart failure (cardiac insufficiency; > Grade I), coronary artery disease (coronary artery disease), cardiac arrhythmias, or constriction in the area of the heart valves)
  • Myocardial infarction (heart attack) less than six months ago.
  • Severe liver failure
  • Acute or chronic lung disease (e.g., pulmonary edema or COPD).
  • Sexually-transmitted diseases such as HIV- syphilis, hepatitis A-D.
  • Persons with erectile tissue implants, anatomical deformities of the penis and acute or chronic inflammation of the foreskin, glans or urethra.
  • Persons taking blood thinners (phenprocoumon).
  • Persons with penile prostheses

  1. MUSE (Medicinal Urethral System for Erection).
  2. SKAT (erectile tissue auto-injection therapy).

Ad 1) Alprostadil is introduced by intracavernosal injection (Caverject and Viridal). Coitus can usually be performed with adequate limb rigidity after onset of action.Success rates are up to 70%. Ad 2) Alprostadil is applied intraurethrally (into the urethra) and to the glans penis (glans) by the patient with the aid of an applicator. Dosing Instructions: Alprostadil should be used only two to three times a week and only once in 24 hours. Onset of action: 5-30 minutes; duration of action: 1-2 hours.

Alprostadil therapy can also be combined with psychological counseling and psychotherapy.
