The term anal fissure describes a tear in the mucosa of the anus. This usually causes severe pain and is mainly caused by strong pressing during bowel movements. In the acute form, conservative treatment is usually sufficient and surgery is not necessary.
There are numerous ointments and creams that support the regeneration of the anal mucosa and can alleviate the symptoms. These have different effects depending on the active ingredients and ingredients and should be used according to the package insert. In case of any uncertainties, it is always recommended to consult a specialist, preferably a proctologist.

In the case of an anal fissure, various ointments can be considered for treatment, most of which are available over the counter. Despite their quick and easy availability, it is important that they are applied correctly. Therefore, it is recommended to inform the patient in the pharmacy.
Further information on correct application can be found in the package insert of the respective ointment. If anything is unclear, a doctor should always be consulted. A medical examination is recommended anyway to clarify the best therapy.
- These include Bepanthen Ointment and Posterisan® acute.
The application of a zinc ointment is rather unusual for an anal fissure. In principle, an ointment containing zinc has a regenerating effect on skin defects. It promotes the healing of wounds and supports various processes that contribute to the decay of the inflammatory reaction.
However, since it can cause irritation of mucous membranes, it is rarely used to treat anal fissures. Furthermore, zinc ointment is preferably applied only on the edges of a miracle. However, since the ointment would spread along the entire anal fissure in the anal region by friction, its use should always be discussed with a doctor or pharmacist beforehand.
The use of Bepanthen ointment is in principle safe in case of an anal fissure. However, there should no longer be a large, open fissure, but advanced wound healing should have already occurred. Bepanthen Ointment promotes wound healing and stimulates skin regeneration due to the precursor vitamin B5 it contains.
Caution should be exercised when using condoms in the genital area, as the ointment can lead to a reduction in the tear resistance of latex. Since Bepanthen Ointment rarely shows side effects, it can be used for several days. DoloPosterine N Ointment is suitable for specific use in the anal region in cases of irritation of the mucous membrane and pain, including anal fissures.
The ointment contains a local anesthetic and thus reduces pain in the applied area. DoloPosterine N Ointment should always be used only after consultation with a physician. DoloPosterine N Ointment should not be used for longer than one week to prevent possible irritation of the anal mucosa.
Ointments with an anal dilator, an applicator or disposable tubes are available for use. A frequently used ointment for anal fissures is the Nitroglycerin Ointment. Usually a concentration of 0.2% is used.
The nitroglycerine contained in the ointment relaxes the sphincter muscle, thus increasing the blood circulation in the anal region. This in turn promotes wound healing and regeneration of the anal fissure. Thus, healing of an acute anal fissure can be achieved within a few weeks.
The Nitroglycerin Ointment is also used repeatedly as an alternative to surgery for chronic anal fissures. It is applied three times a day. The Diltiazem ointment is also suitable for treatment of anal fissures and is usually used in a concentration of 2%.
Diltiazem blocks calcium channels and thus has a relaxing effect on the fibers of the anal sphincter muscles. This results in increased blood flow and promotes wound healing and regeneration of the mucosal defect. The application usually takes place over a period of a few weeks.
The ointment can be applied up to twice a day, after consultation with a doctor. Among the rare side effects are headaches and a lowering of blood pressure. There are different ways of applying the ointment in case of anal fissures.
- For external use, after cleaning the anus and fingers, a thin layer of the ointment can be easily applied and rubbed in with the finger.
- If an anal fissure is treated inside the anal canal, an applicator is included with most ointments.
This can be placed on the tube and filled with the ointment by applying pressure. It is then carefully inserted into the anal canal and the ointment is emptied by pressing the tube. Further information on this can be found in the package insert.
- If necessary, an anal dilator can also be used.
