Cruciate Ligament Rupture: Therapy

General measures

  • In the actup phase, or if an ACL injury is suspected, act according to the PECH regimen:
    • P for pause / interruption of sports activity.
    • E like ice for local cooling of the knee joint
    • C for Compression, i.e. apply a pressure bandage
    • H as elevating the injured limb above heart level.

Medical aids

  • Active motion splint (CAM splint; Controlled active motion); this is a “pedaling machine” that involves both legs; indication: post-treatment for rupture of the anterior cruciate ligament; goal of the procedure: stabilization of the joint, reduction of pain and swelling, and long-term prevention of degenerative changes in the knee
  • Passive motion splint (CPM splint; Engl. Continuous passive motion) for passive movement of the leg; indication and goal: see above.
  • Assessment of the two methods mentioned by the Institute for Quality and Efficiency (IQWiG): Regarding the range of motion and pain, the comparison between CAM and CPM splint shows no relevant differences between the treatment groups; overall, it is pointed out that the study situation is poor.

Physical therapy (including physiotherapy)

  • Muscle training of the quadriceps femoris muscle (four-headed thigh muscle) from the first day after surgeryOne-third of those affected achieve good results with muscle training. One third have to restrict their activities. One-third experience complications.
  • Increasing mobilization after the second week after surgery.
  • Training with active motion splints (CAM, Controlled active Motion): “pedaling machines” that involve both legs; see below “Medical aids”
