Incisional Hernia (Scar Hernia): Surgical Therapy

According to current doctrine, an incisional hernia (scar hernia) should be operated on.

Incisional hernia surgery can be performed as conventional open surgery or laparoscopically (via laparoscopy).

Surgical Procedures

  • Treatment by direct suture; indication: smaller scar hernias (< 2-4 cm).
  • Implantation of a synthetic mesh (open or laparoscopic technique).
    • Sublay mesh position (retromuscular/posterior to muscle).
    • Underlay mesh position (preperitoneal/before the peritoneum).

Postoperative complications and recurrence (recurrence of disease) are dependent on the size of the hernial orifice.

The rate of mesh-related complications is directly proportional to the implanted mesh size.

Approximately 10-14 days after surgery, resumption of normal physical activities such as housework, light gardening, bicycling and driving is possible. Lifting of weights over 10 kg should be done after 6 weeks at the earliest.

Postoperatively, wearing an abdominal bandage for about 6 weeks is recommended. This can already be omitted at night two weeks after surgery.
