Peri-implantitis: Dental Therapy

Conventional non-surgical therapy methods

  • Peri-implantitis prophylaxis:
    • Sealing of implant interiors – reduction of cavities between implant and abutment (implant abutment) that can be colonized by bacteria.
    • Basically periodontal rehabilitation (“healing of diseases of the periodontium/periodontium) before implantation.
    • Risk-oriented screening
  • Elimination of pathogenic microbial flora as a primary cause of disease for peri-implant inflammation:
    • Systemic phase
      • Clarification of general diseases
    • Hygiene phase
      • Optimization of oral hygiene
      • Cleaning of the superstructure
      • Periodontal pretreatment of the residual tooth stock.
      • If necessary, microbiological germ determination
    • Corrective phase
      • Non-surgical pre-treatment
        • Implant cleaning
          • mechanical cleaning
            • Smoothing and polishing – curettes and brushes made of plastic, Teflon or titanium.
            • Ultrasound – only with special working ends; not with remaining osseointegration below 50%.
            • Powder jet device – with glycine and sodium bicarbonate perpendicular to the implant axis, otherwise risk of emphysema formation in the tissue. With sodium bicarbonate, the biocompatibility (body compatibility) of the titanium surface is restored.
        • Decontamination (removal of hazardous substances) of exposed implant surfaces e.g. by citric acid 20% for 30 to 60 sec, followed by NaCl rinsing.
        • Decontamination by laser systems with wavelengths minimally absorbed by titanium (caveat: heating).

          • Photodynamic chemotherapy (PACT) – photochemical interactions between low-intensity laser light and a photosensitizer (photosensitive active substance) with the aim of inactivating germs.
      • Support by oral antiseptics (chemical substances used to prevent wound infection).
      • Support by antibiotics
      • Fixing occlusion disorders (disturbed interaction of the teeth of the upper jaw and lower jaw).
    • Maintenance phase
