- What is the general health of your relatives?
- Are there any diseases in your family that are common?
Social history
- What is your profession?
Current medical history/systemic history (somatic and psychological complaints).
- What symptoms have you noticed?
- Do you suffer from watery/itchy eyes, runny nose, frequent sneezing, coughing irritation, skin changes?
- How long have these symptoms been present?
- Was there a triggering moment for the symptomatology? Contact with animals, latex, etc.?
- When do the symptoms occur? Have you noticed any seasonal dependence?
- How long do they last?
- Have the symptoms worsened?
Vegetative anamnesis including nutritional anamnesis.
- Have you observed allergic reactions to the following food:
- Chicken egg
Self history including medication history.
- Pre-existing conditions (neurodermatitis, frequent viral infections).
- Operations
- Radiotherapy
- Vaccination status
- Allergies
- Environmental history (air pollutants)
- Medication history
