Genital Prolapse: Therapy

General measures

  • Aim for normal weight!Determination of BMI (body mass index, body mass index) or body composition using electrical impedance analysis and, if necessary, participation in a medically supervised weight loss program.
  • Nicotine restriction (refrain from tobacco use).

Medical aids

  • In a later stage of genital prolapse, elevation of the uterus (womb) can be done by pessaries (cube, cup, ring pessaries). However, they should not be used for long-term therapy. Pessary treatment makes simultaneous local estrogen therapy necessary.

Nutritional medicine

  • Nutritional counseling based on nutritional analysis
  • Nutritional recommendations according to a mixed diet taking into account the disease at hand. This means, among other things:
    • Daily total of 5 servings of fresh vegetables and fruits (≥ 400 g; 3 servings of vegetables and 2 servings of fruits).
    • Once or twice a week fresh sea fish, i.e. fatty marine fish (omega-3 fatty acids) such as salmon, herring, mackerel.
  • Observe the following special dietary recommendations for the treatment of constipation (constipation), if any:
    • A diet high in fat and protein (protein-rich) and a high intake of refined carbohydrates promotes constipation.
    • Include high-fiber diet (soluble and insoluble fiber) in the diet plan: the fiber content should be circa 35 g daily. Especially psyllium (psyllium; soluble fiber) and wheat bran (10-30 g / day; insoluble fiber) increase stool weight due to the high water-binding capacity. Flaxseed is also recommended.
    • If there is a potassium deficiency (serum potassium < 3.5 mval/l), this can be remedied by potassium-rich foods (see “food lists”), potassium supplements.
    • Attention should be paid to an adequate amount of fluid, as intake of less than 0.5 liters per day can increase constipation; fluid intake of 1.5-2 liters per day.
    • Diet rich in:
      • Probiotic foods (if necessary, dietary supplements with probiotic cultures).
  • Selection of appropriate food based on the nutritional analysis

Sports Medicine

  • Individually adapted muscle training (pelvic floor training); indication: mild descensus.

Physical therapy (including physiotherapy)

  • For mild genital prolapse, consistent pelvic floor exercises (pelvic floor training) can be started, which may include the use of vaginal cones. These small weights are inserted vaginally and simply worn for as long as the muscles can hold them. Individual pelvic floor training achieved an effect in a randomized trial in the Lancet, of which the affected women were more convinced than the treating physicians.
  • Electrostimulation of the pelvic floor muscles represents another step in the treatment of mild genital prolapse.
