Whiplash Injury: Therapy

The following therapeutic measures can be carried out for “cervical spine trauma grade 1 to 2”. Please note that there is a very slow but steady improvement in this clinical picture.

General measures

  • In the early stages, if necessary” cold compress”; later, local heat or massage
  • Early activating conservative treatment; prolonged immobilization is prognostically unfavorable.

Regular check-ups

  • Regular medical checkups

Physical therapy (including physiotherapy)

  • Physical therapy:
    • Physiotherapy probably provides greater benefit than immobilization by means of a cervical collar (if immobilization, then a maximum of 5-10 days!)S1 Guideline: application of a Schanz collar or other mechanical immobilization (usually superfluous, exception: instability, massive pain on movement)
      • Gentle active movement and loosening exercises.
    • Intensive 12-week physical therapy combined with cognitive behavioral therapy (CBT) did not relieve symptoms of patients with chronic whiplash better than one-hour counseling in a comparative study
  • Massage
  • Dry heat application
  • Electrotherapy

Psychotherapy

  • Psychotherapy – when there is a risk of chronic courses
    • Cognitive pain management therapy
