Amiloride is sold exclusively in fixed combination with the diuretic hydrochlorothiazide in tablet form. The original Moduretic is no longer available in many countries, but generics are available.
Structure and properties
Amiloride is present in drugs as amiloride hydrochloride (C6H9Cl2N7O – 2 H2O, Mr = 302.1 g/mol) is present, a pale yellow to greenish yellow powder that is sparingly soluble in water. It is a pyrazine and guanidine derivative.
Effects
Amiloride (ATC C03DB01) has diuretic and antihypertensive properties. It retains potassium in the body and promotes sodium excretion. The effects last for about 24 hours.
Indications
- For the treatment of hypertension.
- Edema, cirrhosis of the liver with ascites.
Off-label:
- Amiloride is also used off-label as an inhalation for pulmonary disease. In many countries, there are no corresponding drugs and no solutions on the market and there is no approval. Amiloride solutions must be prepared sterilely in a well-equipped pharmacy (eg, hospital pharmacy) ( see under extemporaneous preparations).
