Collagenase From Clostridium Histolyticum

Collagenase from is commercially available as a powder and solvent for the preparation of a solution for injection (Xiapex). It was approved in many countries in 2011. Sales of Xiapex were discontinued in 2020.

Structure and properties

It is a mixture of two collagenases AUX-I and AUX-II obtained by fermentation from the gas gangrene bacterium. The enzymes are polypeptide chains of amino acids with molecular weights of 113 and 114 kDa, respectively.

Effects

The collagenases (ATC M09AB02) dissolve collagen by hydrolysis and allow better mobility. We cannot comment on the actual clinical efficacy.

Indications

For the treatment of Dupuytren’s contracture in patients with a palpable cord. After 24 hours, finger stretching is performed under medical supervision as needed to release the strand. Injection and finger stretching may be repeated a total of three times at one-month intervals.

Dosage

According to the drug label. The drug is injected directly into the affected cord.

Interactions

There is in vitro evidence of inhibitory effects with concomitant use of tetracyclines.

Adverse effects

The most common potential adverse effects include administration site reactions such as bleeding, pain, and swelling; furthermore, lymphadenopathy, pruritus, ecchymosis, and pain in the limbs.
