Perthes Disease: Surgical Therapy

Note

  • Therapy for Perthes disease should usually be conservative (e.g., physiotherapy and self-exercise; positioning elements for sleep).
  • The younger the patient and the lower the Catterall group (see classification below), the more cautious one should be with the indication for surgery.

In Perthes disease, the following surgical therapies are used:

  • Femoral-side procedures such as intertrochanteric realignment osteotomy (synonyms: corrective osteotomy, realignment osteotomy; in which a bone is cut (osteotomized) to produce normal bone, joint, or limb anatomy) or trochanteric dislocation
    • Femoral neck realignment osteotomy results in the head being placed back under the acetabular roof.
  • Pelvic procedures such as the
    • Os innominatum osteotomy or
    • Acetabular osteotomy Note: An osteotomy is a surgical procedure in which one or more bones are specifically cut.
