Quinapril

Products

Quinapril is commercially available in the form of film-coated tablets as a monopreparation (Accupro) and as a fixed combination with hydrochlorothiazide (Accuretic, Quiril comp). It has been approved in many countries since 1989. Generic versions are registered.

Structure and properties

Quinapril (C25H30N2O5, Mr = 438.5 g/mol) is present in drugs as quinapril hydrochloride, a white or faintly pink colored hygroscopic powder that is readily soluble in water. The peptidomimetic is a prodrug and is biotransformed in the body to the active metabolite quinaprilat.

Effects

Quinapril (ATC C09AA06) has antihypertensive properties and unloads the heart (preload and afterload). The effects are due to inhibition of the formation of angiotensin II from angiotensin I by inhibition of angiotensin converting enzyme (ACE). Quinapril thus abolishes the effects of antiogensin II.

Indications

For the treatment of hypertension and heart failure.

Dosage

According to the professional information. The tablets are taken once or twice daily and independently of meals.

Interactions

Drug-drug interactions are possible with diuretics, lithium, NSAIDs, tetracyclines, and antidiabetics, among others. Potassium supplements and potassium-sparing diuretics may increase the risk for hyperkalemia.

Adverse effects

The most common possible adverse effects include cough, low blood pressure, fatigue, drowsiness, headache, pain, insomnia, and indigestion. Other possible side effects include hyperkalemia and angioedema.
