Sinusitis: Or something else? Differential Diagnosis

Respiratory System (J00-J99)

  • Adenotonsillar hyperplasia – enlargement of the tonsils.
  • Allergic rhinitis (common cold)
  • Mucocele – sinus filled with mucus and thus dilated.
  • Pyocele – sinus filled with pus and thus dilated.

Eyes and eye appendages (H00-H59).

  • Eye diseases such as glaucoma

Blood, blood-forming organs – immune system (D50-D90).

Endocrine, nutritional and metabolic diseases (E00-E90).

  • Cystic fibrosis (ZF) – genetic disease with autosomal recessive inheritance characterized by the production of secretions in various organs to be tamed.

Cardiovascular system (I00-I99).

  • Hypertension (high blood pressure)
  • Subarachnoid hemorrhage (SAB; hemorrhage between the spinal meninges and the soft meninges; incidence: 1-3%); symptomatology: proceed according to “Ottawa rule for subarachnoid hemorrhage”:
    • Age ≥ 40 years
    • Meningismus (symptom of painful neck stiffness in irritation and disease of the meninges).
    • Syncope (brief loss of consciousness) or impaired consciousness (somnolence, sopor and coma).
    • Onset of cephalgia (headache) during physical activity.
    • Thunderclap headache/destructive headache (about 50% of cases).
    • Restricted mobility of the cervical spine (Cervical spine).

Infectious and parasitic diseases (A00-B99).

  • Mycosis – fungal infection of the paranasal sinuses – of the paranasal sinuses.

Mouth, esophagus (food pipe), stomach and intestines (K00-K67; K90-K93).

Musculoskeletal system and connective tissue (M00-M99).

  • Arteritis temporalis (synonyms: Arteriitis cranialis; Horton’s disease; giant cell arteritis; Horton-Magath-Brown syndrome) – systemic vasculitis (vascular inflammation) affecting the arteriae temporales (temporal arteries), especially in the elderly.
  • Granulomatosis with polyangiitis (GPA), formerly Wegener’s granulomatosis – necrotizing (tissue dying) vasculitis (vascular inflammation) of the small to medium-sized vessels (small vessel vasculitides), which is accompanied by granuloma formation (nodule formation) in the upper respiratory tract (nose, sinuses, middle ear, oropharynx) as well as the lower respiratory tract (lungs)
  • Cervical syndrome – pain in the area of the cervical spine or complaints originating from the cervical spine area.

Neoplasms – tumor diseases (C00-D48).

  • Benign and malignant (benign and malignant) tumors, unspecified.

Psyche – nervous system (F00-F99; G00-G99).

  • Cephalgia (headache)
  • Cluster headache
  • Meningitis (meningitis), unspecified
  • Migraine
  • Tension headache
  • Trigeminal neuralgia – severe pain originating from the fifth cranial nerve, which primarily supplies the skin of the face

Injuries, poisonings, and other consequences of external causes (S00-T98).

  • Foreign body

Headache (cephalgia) due to/within:

  • Arteritis temporalis (see above).
  • Cluster headache
  • Costen syndrome (dental anomaly)
  • Glaucoma (glaucoma)
  • Hypertension (high blood pressure)
  • Migraine; esp. with unilateral pain in the forehead area; headaches that can be attributed to sinus irritation are migraine-related in 90% of cases
  • Tension headache
  • Trigeminal neuralgia (form of facial pain).
  • Cervical syndrome (discomfort originating in the cervical spine (C-spine)).
