Arm Pain: Or something else? Differential Diagnosis

Congenital malformations, deformities, and chromosomal abnormalities (Q00-Q99).

  • Cervical rib – supernumerary rib that may occur on the fourth to seventh cervical vertebrae.

Cardiovascular system (I00-I99).

  • Aneurysm of the subclavian artery – wall outpouching of the subclavian artery.
  • Angina pectoris (“chest tightness”; sudden pain in the heart area) – due toacute coronary syndrome (ACS), coronary artery disease/coronary artery disease (CAD)).
  • Syphilitic aortitis – form of inflammation of the aorta caused by syphilis.
  • Thoracic outlet syndrome – temporary or permanent compression of a vascular-nerve bundle; there are circulatory disturbances, sensory disturbances, paralysis of the arm.

Infectious and parasitic diseases (A00-B99).

  • Syphilitic aortitis – form of inflammation of the aorta caused by syphilis.

Mouth, esophagus (food pipe), stomach, and intestines (K00-K67; K90-K93).

  • Gastroesophageal reflux disease (synonyms: GERD, gastroesophageal reflux disease; gastroesophageal reflux disease (GERD); gastroesophageal reflux; reflux esophagitis; reflux disease; reflux esophagitis; peptic esophagitis) – inflammatory disease of the esophagus (esophagitis) caused by the abnormal reflux (reflux) of acid gastric juice and other gastric contents.

Musculoskeletal system and connective tissue (M00-M99).

  • Arthritis (inflammation of the joints)
  • Herniated disc (herniated disc) in the cervical spine (C-spine) or thoracic spine (thoracic spine).
  • Epicondylitis humeri lateralis (tennis elbow)
  • Epicondylitis humeri medialis (golfer’s elbow)
  • Frozen shoulder (periarthritis humeroscapularis) – painful frozen shoulder with increasing pain in the shoulder area, at rest and in motion, which occurs during certain movements and sometimes radiates throughout the arm
  • Muscle strain, unspecified
  • Spondylosis (degenerative changes in vertebral bodies (and intervertebral spaces)) of the cervical spine (cervical spondylosis).
  • Subacromial bursitis – bursitis of a bursa in the shoulder region.
  • Tendovaginitis stenosans (Quervain’s disease; “housewife’s thumb”) – tendovaginitis in the thumb area.

Neoplasms – tumor diseases (C00-D48).

  • Malignant neoplasms such as local bone tumors, spine, spinal cord.
  • Malignant neoplasms of the lung, especially Pancoast tumor (synonym: apical sulcus tumor) – rapidly progressive peripheral bronchial carcinoma in the area of the lung apex (apex pulmonis); rapidly spreading to the ribs, soft tissues of the neck, brachial plexus (ventral branches of the spinal nerves of the last four cervical and first thoracic segments (C5-Th1)) and vertebrae of the cervical and thoracic spine (cervical spine, thoracic spine)); disease often manifests with a characteristic Pancoast syndrome: shoulder or Arm pain, rib pain, paresthesia (sensory disturbances) in the forearm, paresis (paralysis), hand muscle atrophy, upper influence congestion due to constriction of the jugular veins, Horner’s syndrome (triad associated with miosis (pupil constriction), ptosis (drooping of the upper eyelid) and pseudoenophthalmos (apparently sunken eyeball)).

Psyche – Nervous System (F00-F99; G00-G99).

  • Carpal tunnel syndrome (KTS) – nerve compression syndrome (narrowing syndrome of the median nerve in the region of the carpal canal); symptoms: Tingling and numbness also pain in the wrist; falling asleep of the hand, especially at night (brachialgia paraesthetica nocturna) [often both hands are affected; risk of confusion with cervical spine syndrome, polyneuropathy (diseases of the peripheral nervous system affecting multiple nerves) or cervical myelopathy / damage to the spinal cord in the cervical spine].
  • Multiple sclerosis (MS)
  • Neuritis nervi ulnaris (synonym: ulnar sulcus syndrome) – the ulnar nerve is palpable in its course on the medial humerus in the elbow region. In this area, compression can be caused by adhesions or muscle parts or stretching of the nerve. Complaints: pain and paresthesias (numbness) in the 4th and 5th fingers; paresis (paralysis) and atrophy of the small hand muscles enervated by the ulnar nerve to the claw position of the ring and little finger (claw hand).
  • Neuritis of the brachial plexus (synonyms: plexus neuritis or neuralgic shoulder amyotrophy/muscle atrophy) – acute inflammation of the brachial plexus associated with severe pain and paralysis of the shoulder and arm muscles.
  • Postzoster neuralgia (PZN) – nerve disease that occurs after herpes zoster (shingles).
  • Subclavian steal syndrome (synonym: vertebral tapping syndrome) – a so-called tapping syndrome. This refers to a condition in which there is blood withdrawal in a specific area as a result of localized blood flow reversal.
